Obesity Clinical Update: 17 July 2026
Orforglipron FDA approval expected next month — UK implications. NHS tirzepatide 6-month outcomes: 13.2% mean weight loss. NICE evidence review on lower BMI thresholds — full draft published.
With the orforglipron FDA PDUFA date one month away, this update covers the UK implications of a potential approval, the NHS tirzepatide 6-month outcomes data, and the publication of NICE's draft evidence review on lower BMI thresholds.
Orforglipron — FDA Decision Imminent: UK Implications
The FDA PDUFA date for orforglipron is 14 August 2026. With no advisory committee meeting scheduled, approval is widely anticipated.
If approved by FDA:
- MHRA decision expected: Q2 2027
- NICE appraisal: expected to begin Q3 2027
- UK availability: potentially 2028, subject to NICE approval and commercial arrangements
Clinical positioning: Orforglipron would be positioned as an oral alternative to injectable GLP-1 agonists, with particular relevance for patients with needle phobia, patients in settings where injectable storage is challenging, and patients who prefer an oral formulation.
Efficacy comparison: Orforglipron's 14.7% weight loss at 72 weeks is comparable to semaglutide 2.4 mg (15.3% in STEP-1) and lower than tirzepatide 15 mg (22.5% in SURMOUNT-1).
NHS Tirzepatide — 6-Month Outcomes
NHS England has published 6-month outcomes data (patients who started 1 February 2026, n=24,800):
- Mean weight loss at 6 months: 13.2%
- ≥10% weight loss: 62%
- ≥15% weight loss: 28%
- ≥20% weight loss: 9%
- Dose distribution at 6 months: 2.5 mg 8%, 5 mg 28%, 10 mg 42%, 15 mg 22%
Comorbidity outcomes at 6 months:
- HbA1c reduction in T2D patients: −14.8 mmol/mol
- T2D remission (HbA1c <48 mmol/mol): 18% of T2D patients
- Blood pressure reduction: −9.4/−5.6 mmHg
NICE — Lower BMI Threshold Evidence Review: Full Draft Published
NICE has published the full draft evidence review on lower BMI thresholds for obesity pharmacotherapy, for consultation until 14 August 2026.
Draft conclusions:
- Pharmacotherapy is clinically effective at BMI 27–29.9 kg/m² with established CVD or T2D (strong evidence from SELECT and SURPASS-CVOT)
- Cost-effectiveness modelling supports extending eligibility to BMI 27–29.9 kg/m² with established CVD (ICER £18,400/QALY) or T2D (ICER £22,100/QALY)
- Lower BMI thresholds for South Asian, Black African, and Black Caribbean populations should be formalised in NICE guidance (−2.5 kg/m²)
Proposed new eligibility criteria (draft):
- BMI ≥35 kg/m² with ≥1 comorbidity (unchanged)
- BMI 30–34.9 kg/m² with T2D, CVD, or high cardiometabolic risk (unchanged)
- NEW: BMI 27–29.9 kg/m² with established CVD or T2D (proposed addition)
