Evidence-based tools, guidelines, and educational resources to support UK healthcare professionals in identifying, assessing, and managing obesity in clinical practice.
Obesity courses for HCPsObesity is a complex, chronic, relapsing, and progressive disease — not a lifestyle choice. The World Obesity Federation and NICE both recognise obesity as a medical condition requiring long-term clinical management. UK healthcare professionals play a critical role in shifting the narrative away from blame and towards evidence-based care.
NICE recommends using BMI as an initial screening tool, with waist circumference used alongside BMI to assess cardiometabolic risk — particularly in people from South Asian, Chinese, and other high-risk ethnic backgrounds. A holistic assessment should include medical history, comorbidities, psychological factors, and readiness to change.
The NHS Tier framework provides a structured approach to weight management: Tier 1 (universal prevention), Tier 2 (community lifestyle programmes), Tier 3 (specialist multidisciplinary services), and Tier 4 (bariatric surgery). Referral criteria and service availability vary across England, Scotland, Wales, and Northern Ireland.
NICE NG238 provides updated guidance on pharmacological treatments for obesity, including GLP-1 receptor agonists such as semaglutide (Wegovy) and liraglutide (Saxenda). These should be used as an adjunct to lifestyle intervention in eligible patients, with clear criteria for initiation, monitoring, and discontinuation.
Weight stigma — negative attitudes and stereotypes about people with obesity — is prevalent in healthcare settings and can deter patients from seeking help. Using person-first language ('person living with obesity' rather than 'obese person'), adopting a non-judgemental approach, and ensuring clinical environments are accessible and inclusive are all recommended by NICE and Obesity UK.
NICE guidelines recommend that psychological assessment and support should be integral to weight management pathways. Cognitive behavioural therapy (CBT), motivational interviewing, and acceptance and commitment therapy (ACT) have evidence for improving outcomes in people with obesity. Integrated care systems (ICSs) in England are increasingly commissioning joined-up physical and mental health services.
