Obesity Clinical Update: 21 August 2026

A landmark week for obesity pharmacotherapy: the FDA approved orforglipron on 14 August, making it the first oral non-peptide GLP-1 receptor agonist for obesity. The NHS tirzepatide programme reaches 200,000 patients, and Eli Lilly confirms retatrutide Phase 3 data will be presented at ObesityWeek in October.

Orforglipron — FDA Approved 14 August 2026

The US Food and Drug Administration approved orforglipron (brand name: Orformin) on 14 August 2026 for chronic weight management in adults with obesity (BMI ≥30 kg/m²) or overweight (BMI ≥27 kg/m²) with at least one weight-related comorbidity.

Approved dosing:

  • Starting dose: 3 mg daily for 4 weeks
  • Escalation: 12 mg, 24 mg, 36 mg, 45 mg at 4-week intervals as tolerated
  • No food or water restrictions (unlike semaglutide oral/Rybelsus)

UK regulatory pathway:

  • MHRA review: underway (submitted simultaneously with FDA)
  • Expected MHRA decision: Q2 2027
  • NICE appraisal: expected to begin Q3 2027
  • UK availability: 2028 (subject to NICE approval and commercial arrangements)

NHS Tirzepatide — 200,000 Patient Milestone

The NHS tirzepatide obesity programme has reached 200,000 patients, just under 7 months after launch on 1 February 2026.

  • Cumulative patients: 202,000
  • Prescribing rate: 9,500 new patients per week
  • Mean weight loss at 6 months (n=24,800): 13.2%
  • Continuation rate at 16-week review: 76%

NHS England has confirmed that the programme will continue to expand, with a target of 500,000 patients by end of 2027.

Retatrutide — TRIUMPH Data at ObesityWeek 2026

Eli Lilly has confirmed that primary endpoint data from the TRIUMPH Phase 3 programme will be presented at ObesityWeek 2026 in Denver, Colorado (12–15 October 2026).

The presentation will include TRIUMPH-1 (retatrutide in obesity without diabetes) and TRIUMPH-2 (retatrutide in obesity with type 2 diabetes). Phase 2 data showed up to 24.2% weight loss at 48 weeks with retatrutide 12 mg.

Regulatory submissions to the FDA, EMA, and MHRA are planned for Q1 2027.

Research Digest

Tirzepatide and fertility: A case series published in Fertility and Sterility reported spontaneous pregnancy in 12 women with PCOS and obesity who had previously been unable to conceive, following 6–12 months of tirzepatide treatment. Prescribers should counsel women of childbearing age about the need for effective contraception.

This update is prepared for UK healthcare professionals. All prescribing decisions should reference current NICE guidance, the BNF, and individual patient clinical assessment.
