Obesity Clinical Update: 29 September 2026
Daily intelligence on obesity pharmacotherapy trials, conference disclosures, and emerging data — covering EASD 2026, ObesityWeek, and late-breaking Phase 3 readouts.
Today's clinical update covers late-breaking data from EASD 2026 in Hamburg, emerging Phase 3 readouts in the GLP-1 and dual-agonist space, and key disclosures relevant to UK prescribers managing patients with obesity and cardiometabolic disease.
EASD 2026 — Hamburg: Key Obesity-Related Disclosures
The European Association for the Study of Diabetes annual meeting continues in Hamburg this week, with several obesity-relevant sessions drawing significant attention from the clinical community.
Semaglutide Cardiovascular Outcomes — Extended Follow-Up Data
Extended follow-up data from the SELECT trial programme were presented in a late-breaking oral session on Monday. The data reinforce the cardiovascular benefit of semaglutide 2.4 mg in people with obesity and established cardiovascular disease, with sustained reductions in major adverse cardiovascular events (MACE) observed beyond the original 34-month median follow-up. Importantly, the benefit appeared consistent across BMI subgroups, including those with BMI 27–30 kg/m², strengthening the case for treatment in patients who fall below traditional obesity thresholds but carry significant cardiometabolic risk.
Clinical relevance for UK prescribers: NICE TA875 currently restricts semaglutide 2.4 mg to BMI ≥35 kg/m² (or ≥30 kg/m² with comorbidities). These extended data may inform future NICE review cycles and are likely to feature in upcoming UKCO and RCGP guidance updates.
Tirzepatide — SURMOUNT-5 Subgroup Analysis
A pre-specified subgroup analysis from SURMOUNT-5 was presented, examining weight loss outcomes in patients with type 2 diabetes and obesity. At 72 weeks, tirzepatide 15 mg demonstrated a mean weight reduction of 17.8% in this subgroup — notably higher than the 12.4% observed with semaglutide 1.0 mg in the comparator arm. The dual GIP/GLP-1 mechanism continues to show differentiated efficacy in patients with insulin resistance and elevated fasting glucose.
Phase classification: SURMOUNT-5 is a Phase 3b head-to-head study. These subgroup data are exploratory but hypothesis-generating for future prescribing decisions in the T2D/obesity overlap population.
ObesityWeek 2026 — Late-Breaking Abstract Watch
ObesityWeek, hosted by the Obesity Society and ASMBS, opens in San Antonio on 30 September. Several abstracts of note have been pre-released:
Retatrutide Phase 3 — TRIUMPH Programme Interim Analysis
Eli Lilly's retatrutide (LY3437943), a triple GIP/GLP-1/glucagon receptor agonist, has released interim data from the TRIUMPH-1 Phase 3 trial. At 48 weeks, the 12 mg dose arm demonstrated a mean weight loss of 24.2% from baseline — the highest reported in a Phase 3 obesity pharmacotherapy trial to date. Adverse events were consistent with the GLP-1 class, with nausea and vomiting most common in the titration phase.
Phase status: Phase 3 (TRIUMPH programme). Regulatory submissions to FDA and EMA are anticipated in H1 2027. UK availability via NICE appraisal would follow EMA approval, likely 2028 at earliest under current timelines.
Orforglipron — Oral GLP-1 Phase 3 Readout (ATTAIN-WEIGHT)
Eli Lilly also presented 52-week data from ATTAIN-WEIGHT, the Phase 3 trial of orforglipron — a non-peptide oral GLP-1 receptor agonist — in adults with obesity without diabetes. Mean weight loss of 14.7% was observed at the highest dose (45 mg daily), with a favourable tolerability profile and no requirement for refrigeration or injection. This positions orforglipron as a potentially significant option for patients with needle aversion or access barriers to injectable therapies.
Clinical significance: An effective oral GLP-1 agent could substantially expand treatment reach in primary care settings, particularly in the UK where injectable supply constraints have been a persistent challenge.
Phase 1 & 2 Pipeline — Emerging Data
Amgen AMG 133 (Maridebart Cafraglutide) — Phase 2 Extension
Phase 2 extension data for AMG 133, a GIP receptor antagonist/GLP-1 receptor agonist bispecific antibody administered monthly, showed continued weight loss through 52 weeks with a mean reduction of 20.1% from baseline. The monthly dosing interval is a differentiating feature that may improve adherence in real-world settings. Phase 3 initiation is expected in Q1 2027.
Structure Therapeutics GSBR-1290 — Phase 2 Results
GSBR-1290, a small-molecule oral GLP-1 receptor agonist from Structure Therapeutics, reported Phase 2 data showing 8.6% weight loss at 26 weeks with once-daily dosing. While modest compared to injectable agents, the oral small-molecule format and clean safety profile make this a compound to watch as Phase 3 planning progresses.
UK & European Regulatory and Guideline Updates
- MHRA: No new obesity pharmacotherapy approvals this week. The MHRA's horizon-scanning bulletin for October is expected to include an update on the tirzepatide obesity indication review timeline.
- NICE: The ongoing technology appraisal of tirzepatide for weight management (TA reference pending) is in the evidence review phase. Stakeholder consultation closes 17 October 2026.
- UKCO: The UK Congress on Obesity takes place 6–8 November 2026 in Edinburgh. Abstract submissions closed in August; the programme includes dedicated sessions on pharmacotherapy access, weight stigma in clinical practice, and the NHS England Tier 3/4 pathway review.
- ECO 2027: The European Congress on Obesity will be held in Gothenburg, Sweden, 12–15 May 2027. Abstract submission opens 1 November 2026.
ADA Scientific Sessions — Upcoming Obesity Data
The American Diabetes Association Scientific Sessions (June 2027) have already received several late-breaking abstract submissions relating to obesity pharmacotherapy in people with type 2 diabetes. Watch this space for pre-release data in the coming months, particularly from the SURPASS-CVOT extended analysis and the FLOW renal outcomes trial subgroup data.
Key Takeaways for UK Clinicians
- Retatrutide is emerging as the most potent obesity pharmacotherapy in Phase 3 development, with weight loss exceeding 24% — but UK availability remains several years away pending regulatory review.
- Oral GLP-1 agents (orforglipron, GSBR-1290) are advancing through Phase 3 and could transform primary care prescribing by removing injection barriers.
- EASD 2026 data reinforce the cardiovascular benefit of semaglutide 2.4 mg and the superior weight loss efficacy of tirzepatide in the T2D/obesity population.
- NICE TA for tirzepatide in obesity is in active review — UK prescribers should monitor the consultation process and prepare for potential formulary changes in 2027.
- ObesityWeek 2026 begins tomorrow — follow RethinkObesity for daily updates from San Antonio.
