Obesity Clinical Update: 27 March 2026
NHS tirzepatide 2-month data: 70,000 patients, early outcomes emerging. REDEFINE-2 CagriSema data in type 2 diabetes. NICE lower BMI threshold consultation closes — key themes.
Two months into the NHS tirzepatide rollout, early real-world outcomes are emerging. Novo Nordisk presents REDEFINE-2 data for CagriSema in type 2 diabetes, and the NICE lower BMI threshold consultation closes with strong stakeholder engagement.
NHS Tirzepatide — 2-Month Real-World Outcomes
NHS England has published the first real-world outcomes data from the tirzepatide obesity prescribing programme.
2-month data (n=70,000 cumulative patients; outcomes available for 26,800 who started 1 February):
- Mean weight loss at 8 weeks: 4.2%
- ≥5% weight loss at 8 weeks: 31% of patients
- Discontinuation at 8 weeks: 8.4% (GI intolerance 4.1%, personal choice 2.8%, other 1.5%)
- GP satisfaction with prescribing framework: 78% "satisfied" or "very satisfied"
Early clinical observations:
- Patients with T2D showing earlier and more pronounced HbA1c improvements than expected
- Blood pressure reductions evident at 8 weeks in hypertensive patients
CagriSema — REDEFINE-2: Type 2 Diabetes Data
Novo Nordisk presented 68-week data from REDEFINE-2 (CagriSema in adults with obesity and type 2 diabetes, n=2,400) at the ENDO 2026 meeting in Chicago.
Key results:
- Mean weight loss: 20.8% CagriSema vs 9.4% semaglutide alone vs 2.1% placebo
- HbA1c reduction: −18.2 mmol/mol CagriSema vs −11.4 mmol/mol semaglutide alone
- T2D remission (HbA1c <48 mmol/mol off medication): 32% CagriSema vs 14% semaglutide alone vs 3% placebo
NICE Lower BMI Threshold Consultation — Key Themes
The NICE consultation closed on 3 April 2026. Key themes from the 1,240 responses received:
- Broad support for lower thresholds: 84% of clinical respondents supported extending eligibility to BMI 27–29.9 kg/m² with cardiometabolic risk factors
- Ethnic minority thresholds: Strong support (91%) for formalising lower BMI thresholds for South Asian, Black African, and Black Caribbean populations
- Cost-effectiveness concerns: Several ICBs raised concerns about the cost implications of extending eligibility
NICE will publish its full evidence review by Q4 2026.
